The Open Enrollment Period is from November 1st through January 31st.  However, if Your prior coverage ended for a reason other than non-payment, then You will qualify to apply within 60 days under Special Enrollment provision.

To apply for health insurance, please email the following documents to varshamins@sbcglobal.net

• The 1040 pages of Your taxes
• US Passports or Green Cards or Birth Certificates or Work Authorization cards
• A VOID check
• Letter of Termination, in case You are applying outside the Open Enrollment Period

We offer both PPO and HMO plans with Blue Shield, Blue Cross, LA Care, Health Net.  We always automatically check if You qualify for tax credits from Covered California.

The Rates are dependent on Your zip code, income and age.
